Privacy Policy
TeleWell values your privacy. TeleWell offers a telehealth platform for users seeking access to mental health services.Information about users and their treatment choices are maintained in the telehealth platform. Any website or Internet service under the control of TeleWell, whether partial or otherwise, in will be referred to as the “Platform.”
The Privacy Policy explains how we collect, use, disclose, and secure personal information we obtain about you through the Platform. While TeleWell takes the steps described in this policy to protect your information, TeleWell is not regulated by HIPAA and makes no guarantee that the Platform is compliant with HIPAA. The terms “Company,” “we,” “our,” and “us” include TeleWell Inc and our affiliates and subsidiaries. This Privacy Policy is incorporated by reference into our Terms of Use. Any capitalized terms used and not defined in this Privacy Policy shall have the meaning given to them in the Terms of Use.
WHAT INFORMATION WE COLLECT AND WHY
Information you provide to us directly. We collect personal and health information you provide directly to us through the Platform. For certain activities, such as when you (i) register to use the Platform, (ii) use the Platform, including to receive telehealth services, (iii) make payments through the Platform, or (iv) communicate with us through the Platform, we may collect personal, demographic, health, payment and/or other miscellaneous user information such as:
• Name, date of birth, address, email address, phone number, photograph of your ID 

• Username and password 

• Your, race, ethnicity, sex, emergency contact information 

• Any other information you provide, such as, relationship status, height, weight, religious preferences, emergency contact information, presenting problem, payment information and photograph of your ID 

We may combine such information with additional information we receive from you outside of the Platform, including through phone calls, video calls, or text messages.
The Platform also provides an opportunity for you to input your notes, observations, thoughts, or opinions (“Journal”). We collect all information you provide in your Journal and do not recommend inputting sensitive personal information into your Journal because we cannot be absolutely sure that it is adequately protected. Your journal is accessible and viewable to you and your assigned provider.
Information you provide for payment processing. Our payment processors may collect payment information you provide (such as a credit card number, expiration date, credit card security number, or bank account number) as necessary to help you complete a purchase. TeleWell does not have access to information submitted to our payment processors. We recommend reviewing the payment processors’ privacy policies as they govern the use and collection of information you provide to them.
Information we collect from your health care provider. Your health care provider may choose to input notes, observations, thoughts, or opinions about you into the Platform (“Therapy Notes”) in order to perform their services. Therapy Notes are not accessible or viewable by you in the Platform. Please contact your health care provider directly for any requests related to Therapy Notes.
Information we collect automatically.
When you visit our Platform or open our emails, we and our third-party partners, such as analytics providers, collect certain information by automated means, such as cookies, web beacons and web server logs. By using the Platform, you consent to the placement of cookies, beacons, and other similar technology in your browser and on emails in accordance with this Privacy Policy. The information collected in this manner includes IP address, browser characteristics, device IDs and characteristics, operating system version, language preferences, referring URLs, and information about the usage of our Service. We may use this information, for example, to determine how many users have visited certain pages or opened messages or newsletters, or to prevent fraud. We may link this data to your profile. Our partners also may collect and combine information collected on our Platform and emails with other information about your online activities over time, on other devices, and on other websites or apps, if those websites and apps also use the same partners. These information collection technologies may also be used to display advertising content or services that we think will be of interest to you based on your use of the Platform. You may be able to change browser settings to block and delete cookies when you access the Platform through a web browser. However, if you do that, the Platform may not work properly. The Platform does not respond to browser do-not-track signals.
How DOES TeleWell USE AND DISCLOSE YOUR INFORMATION
We will use and disclose the information we receive from and about you to provide you with access to the Platform and to improve our products and services in accordance with applicable law. For example, we may disclose the information to our vendors so they can provide their services to us, including (i) verifying your identification during the registration process, and (ii) processing payments for selected treatment options. We may also disclose information to your health care provider who accesses the platform as part of their services. We may store and use the information we receive from and about you to contact you to provide support or for TeleWell marketing and promotional purposes by various means, including mail, email, telephone, including voicemail, or SMS/MMS (text message). We may create, use, and disclose de-identified information (that is, information that cannot be used to identify you) or aggregated information for our internal business purposes in compliance with applicable law.
We may also use, preserve, and disclose information (as permitted by applicable law):
• if we believe doing so is required or appropriate to: comply with law enforcement requests and legal process, such as a court order or subpoena; respond to your requests; comply with the law; and protect your, our, or others’ rights, property, or safety; 

• if we are involved in a merger, acquisition, financing due diligence, reorganization, bankruptcy, receivership, sale of TeleWell assets, or transition of service to another provider, your information may be disclosed in connection with the negotiation of such transaction, and/or sold or transferred as part of such a transaction as permitted by law and/or contract; and 

• for the purpose you provide it; 

• with your permission. 

LINKS TO THIRD PARTY WEBSITES
The Platform may contain links to third party websites. This Privacy Policy does not apply to the practices of other websites, and we are not responsible for the actions and privacy policies of the third parties that operate or interact with those other websites.
INFORMATION SECURITY
TeleWell takes a variety of technical and organizational security measures to protect your information against accidental or unlawful destruction or accidental loss, alteration, unauthorized disclosure or access. However, no method of transmission over the Internet, and no means of electronic or physical storage, is absolutely secure. As such, you acknowledge and accept that we cannot guarantee the security of your information transmitted to, through, or on our Platform or via the Internet and that any such transmission is at your own risk. Where we have given you (or where you have chosen) a password that enables you to access the Platform, you are responsible for keeping this password confidential. We ask you not to share your password with anyone.
UPDATE YOUR INFORMATION OR POSE A QUESTION OR SUGGESTION
If you would like to request that we update or correct any account information that you have provided to us through your use of the Platform, or if you have suggestions for improving this Privacy Policy, please send an email to Support@TeleWellApp.com. You may also update or correct your account information in your profile within the Platform.
CHANGES TO OUR PRIVACY POLICY AND PRACTICES
We may make changes to this Privacy Policy from time to time, and the revised version will be effective when it is posted. If we make any material changes, we will let you know through the Platform, by email, or other communication. We encourage you to read this Privacy Policy periodically to stay up-to-date about our privacy practices. As long as you use the Platform, you are agreeing to this Privacy Policy and any updates we make to it.
